Identification et confidentialité

Dans le cadre de l’application de la Charte de la personne hospitalisée et de la Loi du 4 mars 2002 relative aux droits des patients et à la qualité du système de santé, un bracelet d’identification vous sera posé lors de votre admission dans le service de soins, vous le conserverez tout au long de votre séjour dans notre établissement.

L’objectif de ces bracelets est de renforcer la sécurité d’identification du patient aux différentes étapes de sa prise en charge dans le respect des règles de confidentialité.

Dans le cadre de l’identitovigilance, avec votre autorisation, votre pièce d’identité sera insérée dans votre dossier patient informatiCe document est destiné à une utilisation exclusivement professionnelle, il ne sera ni communiqué à d’autres personnes, ni vendu, ni utilisé à d’autres usages, dans le respect de la Loi Informatique et Libertés, avec l’autorisation de la Commission Nationale Informatique et Libertés (CNIL).

Votre dossier est conservé par l’établissement à l’issue de votre hospitalisation selon la réglementation en vigueur (cf. dans le "livret d'information patient", chapitre « Informations complémentaires en vue de votre intervention », rubrique « Accès à votre dossier médical »).

Practical Information

Your stay

Identification and confidentiality

As required by the Charte de la personne hospitalisée and the Loi du 4 mars 2002 relative aux droits des patients et à la qualité du système de santé, you will be given an identity bracelet at the moment of your admission.  You must wear it througout your stay in the clinic.

The purpose of these bracelets is to facilitate the identification of the patient at the various stages of his or her care while respecting the rules of confidentiality.

The rooms

All the rooms are equipped with functional furniture, a bathroom, a television (with supplement - see the booklet "Practical information"), a telephone with a direct line (with supplement - see the booklet "Practical information") and a system for calling the nursing staff.

If you want a private room (for a supplementary fee - see the booklet "Practical information") you should request it when booking your procedure at surgeon's office. Your request can only be granted in function of your pathology and especially in function of the availability on the day of your admission.

You can, if you wish, request a meal and / or an accompanying bed (subject to availability and a supplementary fee - see "Practical information" sheet). The accompanying bed can only be requested in the case of a private room and in function of that room's size.  Thus a person of your choice, unless there is a medical contraindication, can accompany you during your stay.

Some complementary health or insurance policies may contribute to these costs (please contact your organization).

For safety reasons, the rooms of the Continuous Surveillance Unit are equipped with an individual video surveillance monitor. 

 

Toiletries and personal hygiene

You must bring your personal linen (dressing gown, pajamas, underwear, slippers, ..) and your toiletries (toothbrush, toothpaste, razor, soap, comb, towels, gloves, ...) . You are responsible for your laundry.

Good personal hygiene is essential for your health, comfort and well-being. Remember to wash thoroughly before your admission, as you will help prevent the risk of infection associated with treatment. 

In the Intensive Care Unit and in the Cardiological Intensive Care Unit, only toiletries are authorized. In the event of transfer to one of these services, your family may bring your personal effects.

 


Pharmaceutical Treatment

When you are admitted, we request that you bring with you all of your current medication, along with the corresponding prescriptions, so that the nursing staff can verify that no medication taken in the days preceding your hospitalization has contra-indication with the procedure you are going to undergo.

This medication should be handed to the nursing staff (legislation prohibits the patient from keeping medicines in their room; indeed any self-medication can lead to the risk of overdose and interactions).  In signing the patient entry booklet (Livret d'Entrée Patient) you agree to not keep any medication with you.

The pharmaceutical treatment prescribed to you during your stay is supplied by the hospital's internal pharmacy.  The use of medication other than that issued by the hospital is forbidden.  However, if any medication from your external prescription is unavailabe in the hospital pharmacy, and if not sutable substitute is available, we reserve the right  to use your personal medication until the pharmacy is resupplied.


Telephone 

Each bed (with the exception of the beds in the Continuous Monitoring Units and the Outpatient Surgery Unit) has a telephone.  It is only possible to receive calls.


Television

All the rooms in the inpatient departments are equipped with a television (at an additional cost - see the booklet "Practical information").  Be sure to respect the tranquility and rest of other patients by moderating the sound level of the television.  A single-use TV headset (on sale at reception) is compulsory in a double room.


Meals

Meals are prepared and cooked according to the most modern procedures (the HACCP* method), guaranteeing perfect hygiene and dietary monitoring at all stages of the chain.  They are determined in consultation with your practitioner and the dietitian, depending on your state of health.

Meals are served in the rooms by nursing assistants and hospital service employees at the following times :

  • Breakfast from 7:45 a.m.
  • Lunch from 12 noon
  • Dinner from 5:45 p.m.

If you are admitted to the outpatient surgery unit, you will be served a snack before your departure.


Movement in the clinic

 

For everyone's benefit, silence in the corridor is essential. Wearing a dressing gown or respectable attire is compulsory outside the bedroom.

We ask you to inform the nurse of any absence from your room because additional examinations may have been planned for you.

For health and security reasons, we ask you to not stay in the reception area, and to respect any indicated access restrictions.


Religion

 

You can receive a visit from a minister of religion of your choice.  Please ask the nursing staff.


Damage and theft

 

Any theft or damage caused by the patient or their visitors during their stay are their responsibility and the amount of repairs, replacements or damage will be charged to their invoice.

For your safety, the establishment is under video surveillance (Loi n° 95 – 73 du 21/01/95 et Décret n°96 – 926 du 17/10/96).


Visits

To facilitate your care and upkeep of your room, visits are authorized every day, unless against medical advice:

  • from 12 noon to 9 p.m. in surgery, medicine and maternity
  • from 1 p.m. to 3 p.m. in the Continuous Monitoring Unit (USC)
  • from 12 noon to 1 p.m. and from 5 p.m. to p.m. in  Cardiological Intensive Care A (only 1 visitor per patient)
  • from 3 p.m. to p.m. in Cardiological  Intensive Care B
  • from 1 p.m. to p.m. and from 7 p.m. to 8:30 p.m. in Intensive Care, with medical approval (3 visitors maximum per patient)

For reasons of hygiene and for their own safety, visits by young children (up to 5 years old) are not allowed.

As well-being and comfort are essential for the health of hospitalized patients, it is advisable to limit the number of visitors per room (no more than 2 people).

We ask visitors to please leave the rooms during treatment.

If you do not want your presence in the establishment to be disclosed, it is essential to warn the Direction des Soins or nursing staff, and to inform to the Admissions Office (Bureau des admissions) as a priority: your anonymity will be respected.


Money and valuables

We advise you to bring only what is strictly necessary (see the booklet "Prictical Information")  and not to keep your valuables with you during your hospitalization (money, jewelry, check books, securities, bank cards...).

The rooms are equipped with individual safes. We strongly recommend that you store your valuables and papers there. The management accepts no responsibility for your belongings, other than those placed in the safe.

We kindly ask you to bring with you only the amount of the deposits that will be requested from you upon your admission, as well as the means of payment necessary for the full payment of your expenses upon your discharge.


Prostheses

We ask that you take care not to lose your personal items, and in particular your hearing aids, dentures, glasses and contact lenses. You can entrust them to a member of your family during times when they are not essential to you.

If you want to keep your prostheses and / or glasses during your stay, consider bringing a storage box. We strongly recommend that you do not place them in a handkerchief, a compress or any other medium likely to be thrown away.

The hospital accepts no responsability for any loss or damage to your hearing aids, dentures, glasses and contact lenses during your stay, given this information and the presence of a safe in your room.


Animals

 

For hygiene reasons, animals are strictly prohibited in ALL the establishment.


Flowers and plants

 

It is always a pleasure to give or receive flowers.  However, for reasons of hygiene, they are not recommended.

Plants are prohibited as soil can be a source of microbial contamination.

Flowers and plants are strictly forbidden in the Continuous Monitoring Unit, in Intensive Care Unit, in the Cardiology Intensive Care Unit and in the Outpatient Surgery Unit.


 

Your Discharge

Formalities

The clinic respects your freedom to leave. However, only the Doctor responsible for your hospitalization can decide when you will be discharged.

We kindly ask you to complete the following formalities:

  • Depending on your type of hospitalization, wait for the agreement of the surgeon or the practitioners who took care of you (anaesthetist, gynecologist-obstetrician, pediatrician): you should only leave the establishment with their agreement.  Otherwise, if you want to leave against medical advice, you will have to sign a release that discharges the clinic and the practitioners from any responsibility for any possible consequences.
  • For the discharge formalities, you must go to the Admissions / Discharge Office (except for patients in the Outpatient Surgery Unit):
  • You will be given your discharge documents, including the bulletin de situation specifying the duration of your stay, that you should send to your employer and / or insurance.
  • You will need to pay for any costs for which you are responsible (see the details in the "frais de séjour" section) and recover your deposit after the full balance has been paid (reimbursement  by the Social Security and  / or your health insurance) .
  • The surgeon may request a check-up within a variable period and possibly post-operative care: in this case, you will be informed of the scheduled appointments before your departure.
  • If the hospitalized person is a minor, the person responsible for parental authority or the designated person (cf. Patient Entry Booklet) must be present, having with them their identity document (Décret du 17/ 01/1974).
  • If your state of health, confirmed by a medical prescription, requires you to be transported by an ambulance or a light medical vehicle, to your home, to a convalescent home (indicate upon admission) or for rehabilitation (in consultation with the surgeon), you are free to choose the company (a list of transporters approved by the DDASS is available in the discharge office).
  • Certain doctors practicing within the clinic are authorized, by virtue of their sector of affiliation, to charge dépassements d’honoraires, that is fees in excess of those approved by the Assurance Maladie . Check with the doctor.

To help with or organization, we ask yout o vacate your room between 9:00 am and 11:30 am. on the day of your discharge.  You will need to return your TV remote control in working order. Make sure not to forget anything in the room when you leave (medical or radiological documents, personal items, etc.).

The personal documents that you brought during your admission will be returned to you: examinations (biological, medical imaging, cardiological ...) and blood group card.

To allow the continuity of your care, certain documents will be given to you at the time of discharge: prescriptions, hospitalization report, liaison letter, nurse discharge document and a practitioner's letter addressed to your family physician (may be sent later).


Hospital fees

The costs inherent to your hospitalization and the care required by your state of health are paid directly to the clinic by the Social Security.

The services not covered by the Assurance Maladie, your complementary Insurance, or your health insurance, such as the daily fee, the flat-rate contribution of 24 €, the co-payment, the supplements to doctors' fees, any options you have requested (private room, telephone, television, accompanying bed, hotel services...) remain at your expense.

You must pay the daily fee, the amount of which is set by the government. This fee is not included in the establishment's budget.

There exist certain exemptions from the daily fee, examples of which are:

  • Beneficiaries of Article 115 du Code des Pensions militaires (war pensioners)
  • CMU
  • Outpatients
  • Workplace accidents
  • Régime local Alsace Lorraine

If you have any form of health insurance you should present your certificate (attestation) or your health insurance card to ensure that you benefit.

If you are not covered at 100%, the law imposes a participation corresponding to the amount of the ticket modérateur (the part of the treatment not reimbursed by the Assurance Maladie)  (i.e. 20% of the liability rate of the Social Security).

Some complementary health insurances cover or reimburse this ticket modérateur;  Adherants to such contracts are advised to ask their insurance provider or bring their contract.

Some procedures are reimbursed bythe Assurance Maladie at 100% and are thus exonerated from a ticket modérateur.  Exemples are:

  • Certains procedures (ask your surgeon)
  • CCAM acts (or procedures) ≥ 120 € and NGAP acts ≥ à 60 €
  • Workplace accidents
  • Beneficiaries of Article 115 du Code des Pensions militaires (war pensioners)
  • Régime local Alsace Lorraine
  • Invalidity
  • Care related to an illness that the social security reimburse at 100%

If you do not benefit from any social security coverage or if your coverage has not been accepted, you will have to pay the full cost of the stay.

Hospitalization costs can be paid in cash, bank checks or credit cards.

 

Coverage of transport costs by health establishments

As set out in décret n° 2018-354 du 15 mai 2018, the methods of covering the expenses of transporting hospitalized patients or on permission to leave by health establishments have changed.  Until then limited, the scope of transport expenditure payable by health establishments changed the 1st October, 2018.

If you are hospitalized in a health establishment in medicine, surgery or obstetrics from October 1, 2018, any transport prescribed for a transfer between 2 health establishments (including from or to chemotherapy, radiotherapy, dialysis centers ) is organized and paid for by the prescribing health establishment. If during hospitalization, transport is prescribed to you for a leave of absence for personal convenience, the transport will be at your expense. For transport that has been prescribed to you for radiotherapy sessions carried out by a private structure or a healthcare center, you have a choice of  transport company and this transport will be reimbursed by the Assurance Maladie, subjet to the usual conditions.
